SMS, WhatsApp or email? A channel matrix for aesthetic clinic communication

Confirmations, reminders, aftercare and newsletters are different messages. Here is which channel fits each one, what it costs in 2026 and when you need consent.

A clinic sends several very different kinds of messages: booking confirmations, reminders, aftercare instructions, invitations to the next session in a series, newsletters about a new treatment. Most clinics push all of them through one channel, usually SMS, simply because that is how it has always been done. Yet according to CBOS (2026), 69% of Polish adults now chat through a messaging app, and since July 2026 sending WhatsApp messages to Polish numbers costs noticeably less than a year ago. Channel choice is no longer obvious, and it is worth making deliberately, separately for each message type.

Where your clients actually are

Data from the last two years shows a clear trend: messaging apps have become an everyday conversation channel, but email and SMS have not gone away.

  • Messaging apps: in the month before the CBOS survey, 85% of internet users chatted through a messaging app, which is 69% of all adults. That is the highest result in the survey's history, and among people under 35 it reaches 94–99% (CBOS, 2026).
  • App awareness: in the UKE consumer survey, 94% of respondents knew Messenger and 88% knew WhatsApp. Email remained the most used internet application (84%) (UKE, 2023).
  • Mobile phones: Poland has 53.8 million active cellular connections, equal to 142% of the population (DataReportal, 2026). SMS reaches practically everyone, regardless of age or apps installed.

The practical takeaway: SMS has the widest reach, a messaging app is a conversation channel, and email is the channel for anything longer than a few sentences. Each has a different job.

Unit cost is easy to ignore until you multiply it by several thousand appointments a year.

Since 1 July 2025, Meta charges per delivered template message rather than per 24-hour "conversation". Utility messages sent inside an open customer service window (after the client has written or replied) are free. Since 1 July 2026, Poland has its own market-specific rates in Meta's rate card instead of regional pricing, lower for marketing, utility and authentication messages (Meta for Developers, 2026).

Consent: what you can send without asking

Poland's Electronic Communications Law (PKE) has applied since 10 November 2024. Its Article 398 prohibits using phones, SMS and email for direct marketing without the recipient's prior consent. The UKE President can impose a fine of up to 3% of the previous year's revenue or PLN 1 million (Infor, 2024). Collecting consent separately for each channel is good practice.

In practice, a clinic should distinguish two categories:

  • Service messages tied to a booked appointment: confirmation, reminder, rescheduling, aftercare instructions. They do not promote the offer, they deliver the service.
  • Marketing messages: a new treatment, a discount, a win-back invitation, a newsletter. These require consent for the specific channel.

The line is thin. A reminder with "20% off your second treatment" added becomes a marketing message. If you are unsure about specific wording, check it with a lawyer or your data protection officer.

Reminders work, but not always the way you assume

A 2026 meta-analysis in the Journal of Hospital Management and Health Policy (10 studies, 8,236 patients) found that reminders improve attendance by 11% on average (RR 1.11). Interestingly, the SMS subgroup effect (RR 1.14) did not reach statistical significance, while the phone reminder subgroup did (JHMHP, 2026).

That does not mean SMS fails. It means that sending a reminder is not enough on its own. Timing, wording and the ability to confirm or reschedule with a single tap all matter. A reminder the client cannot reply to is just information, not a no-show reduction tool.

Automation over mass sends

The highest return comes from messages triggered by client behaviour, not one-off blasts to the whole list. In Klaviyo's 2025 Benchmark Report, based on global ecommerce data, automated flows generated up to 30x more revenue per recipient than campaigns: an abandoned cart flow averaged USD 3.07 versus USD 0.10 for an email campaign. The gap between an average and a top automation is also large.

These are online store numbers, not clinic numbers, so treat them as directional: a well-timed, relevant message beats yet another mass promo SMS.

Use the matrix below as a starting point. The rule is simple: pick the channel based on the message's purpose, urgency and legal basis, not on habit.

Rolling out the matrix in 5 steps

  1. List every message the clinic currently sends manually or automatically. Most clinics end up with 8–15 types.
  2. Tag each one as service or marketing. Send marketing only to people with consent for that channel, and record the date and source of each consent.
  3. Assign a primary and a fallback channel using the matrix. If a client has no WhatsApp or does not respond, the system falls back to SMS.
  4. Calculate the annual cost. Example: 4,000 appointments a year × 2 messages (confirmation and reminder) = 8,000 SMS. At PLN 0.16 net that is PLN 1,280 net a year, at PLN 0.10 it is PLN 800. Compare that with the revenue from a single missed appointment.
  5. Measure responses, not sends. The key metrics are confirmation, reschedule and no-show rates, tracked separately for each channel.

FAQ

Does an appointment reminder need marketing consent?

A reminder about a booked appointment with no promotional content is a service message, not direct marketing. Consent under Article 398 PKE is needed once a message promotes your offer. If you are unsure about specific wording, have it checked by a lawyer.

Is WhatsApp cheaper than SMS?

It depends on the message type. A utility message inside an open service window is free; outside it costs USD 0.0122. A marketing message costs USD 0.0366. An SMS through SMSAPI costs PLN 0.10 to 0.16 net. For reminders the difference is small; for two-way conversations with clients WhatsApp often comes out ahead.

Should we drop SMS in favour of a messaging app?

No. SMS has the widest reach and needs no app. A better approach is a messaging app as the primary channel for clients who use it, with SMS as the fallback.

How often should we send a newsletter?

There is no single number. Matching content to the segment matters more than frequency: a client finishing a laser series needs a different message than someone who came in once for a consultation.
